Child's Name - This helps us track unique members to give a more accurate membership count. With your permission, we will list each member's first name and last initial on Our Members page. We will not include last names on our Web site.

City/State - We request this information to see how far-reaching our program is. We may include locations on our Web site, but we will never list hometowns in connection with individual kids. We will always protect the identity of our kids.

Email - This will allow us to contact you to answer your inquiries. With your permission, we may send you information on upcoming projects. We expect to send no more than one email per month. You may opt-out of email contact at any time.
